Unison Diabetes Education Program

Unison Health and Community Services provides free individual counseling and group education classes for people with Type 2 diabetes and pre-diabetes. Services include lifestyle counseling, education on diabetes medications, guidance on insulin initiation and titration, instruction on glucometer usage, provision of glucometer supplies subject to availability, diabetes foot assessment and education, and referral to free diabetes eye screening. The team consists of Registered Nurses and Registered Dietitians. Multiple locations. No referral required.
